Resources for Families and Caregivers

At Summit Future Foundation, we provide families with resources for next steps. What does life look like after schooling ends for your child? What are important goals to continue?

We partner families to create a vision for their child's future, understand how to navigate resources, obtain support to achieve that mission, and develop a tactile plan that they can share with others involved in their child's care. This will give your child the opportunity to live a meaningful and fulfilling life.

Transition Planning Parent Mentoring

Families and caregivers are supported through our five-part transition planning parent mentoring program.

Transition planning parent mentoring supports families of individuals between the ages of 14 - 22, who are in the process of transitioning from school to adulthood.

The course covers 6 core topics including:

  • Creating a vision for your child's future and structuring a meaningful day

  • Establishing an integrated support system

  • Employment planning and other day time engagement opportunities

  • Benefits planning and management

  • How to incorporate your goals for your child's future in their IEP and ISP

  • Housing and Supported Living

The result of the transition planning parent mentoring program is for parents and caregivers to be better equipped to support their child in reaching their fullest potential, by living their most independent and self-directed life.

I feel much more confident in my ability to advocate for my child’s future and the futures of the kids I care for. 

What’s Next Workshop

This one-day seminar will focus on the basics preparing your child for life after high school. Whether your child is 12 or 21, we will be sharing practical tips that you can use to prepare for life after your child ages out of school-based services.

We will discuss how to structure a meaningful day, how to establish formal and informal support, how to understand and navigate benefits planning and management. Participants will leave with a framework and actionable tools that they can use to begin the process of creating a transition plan for their child.

Safely Social Training Series

 

In partnership with Summit Future Foundation, JP Consulting is excited to present Safely Social Parent Workshops aimed to support caregivers of teens in navigating many of the personal and social issues that adolescents, teens and young adults face.

Four Core Topics will Include:

  • Social Media Safety

  • Personal Safety

  • Navigating Puberty

  • Preparing for and having "The Talk" and developing healthy personal relationships
