Watch Me Thrive

This innovative program takes a unique approach to supporting an individual in career planning and exploration as well as developing work readiness skills through work-based learning experiences. Watch Me Thrive also simultaneously supports a family in developing a tactical transition plan that includes trajectory development, identifying support needs, and connecting to local, state, and national support systems.

The program results in individuals and families who are better prepared for the transition from school-based services to adulthood.

Watch Me Thrive is offered to young adults between the ages of 16 - 22 and their families or other caregivers benefit the most from this program.

The frequency and duration of Watch Me Thrive are determined by the person and their support team.

Scheduling is agreed-upon to support the person-centered plan. Summit Future Foundation strives to provide opportunities to engage in work-based learning opportunities at community-based businesses that align with their self-stated employment goals.

  • 2 days - 5 days per week 

  • 2 hour - 4 hour sessions

  • 2 week , 6 week and 10 week tracks

Frequently Asked Questions for Watch Me Thrive

  • As a contracted provider through the Missouri Department of Mental Health, our Home and Community Services are primarily funded through Medicaid waivers, other state funding, such as Missouri Autism Project, or County Support Board funding.

    Families may elect to privately pay for these services, however, due to contracted provider requirements the private pay rate must align with rates for contracted services.

    Due to the likely financial hardship this may cause, our organization is dedicated to supporting families in systems navigation that helps them access funding through the state and other supports.

    In addition, as a show of dedication to this program in 2020 Summit Future Foundation established the Watch Me Thrive Scholarship Fund that ensures all have access to this life-changing program.

  • Individuals seeking support or their family members may self-refer to participate in our programs.

    Targeted case managers through county support boards may also refer a person or their family to our organization.

  • In collaboration with the person, and others that may support them, such as; their parent’s caregivers and case management team, goals and priorities are established that include assisting the person in setting employment-related goals.

    The parent engagement portion of Watch Me Thrive is a curriculum developed from research that documents elements that lead to integrated outcomes for persons with developmental differences.
